What Causes Recurrent Fever?

My son is 18 years old and has had a fever for the past 24 hours that started at 100.9 went up to 103.2 with brief decreases due to Advil that I gave him? I had him sit in a cool bathtub in room temperature water, but what else can be done and when should I take him to the ER?

The condition here is of high grade fever that may be due to following conditions
1.Upper respiratory infection
2. Lower respiratory infection
3. Enteric fever
4. Urinary tract infection
5. Malaria and Dengue( depending on region)
Please look for the symptoms accordingly to localise the cause of fever
For treatment the following must be done
1. Cold sponging
2. Plenty of oral fluids
3. Tab Paracetamol 650 mg thrice daily till fever subsides followed by SOS
4. Inj PCM 1000 mg if fever is continuously above 102 deg Fahrenheit
Look for following signs for immediate contact to ER
1. Persistent high grade fever > 102
2. Evidence of vomiting, pain abdomen , chest pain , persistent cough, burning micturition,headache or any evidence of rash or bleeding
