What Causes Rectal Bleeding And Back Pain In An UTI?

I have been taking medicine for a uti for three days now (nitrofurantoin). Today I felt a lot better and decided to go for a run. About 3 minutes into the run a started getting shooting pain in my abdomen. About a quarter of a mile in i had to turn around and go back home. Once there I discovered a large blood stain in my underwear. I also have lower back pain. Has the infection spread to my kidneys? Should I go to the hospital?
