What Causes Prolonged Heavy Vaginal Bleeding With Clots?

I have been bleeding heavy and light for the last seven months. Nonstop. I get these huge jelly globs that fall out everyday multiple times. Lately the bleeding has been heavier and worse along with a lot of cramping. I ve been to the emergency room and they gave me hormones for 10 days the bleeding stopped for a few days but it is now back worse than ever.
