What Causes Postpartum Abdominal Cramps And Irregular Periods?

I m barley 5 months postpartum. I have had bad cramping , pellvic pain and irregular bleeding had Merna IUD put in march still same problems as far as I know not pregnant. I m sitting on the couch and felt a gush of fluid enough to soak my panties pants and couch. Severe cramping more so then usual. There is no smell to it. It is clear in color. I had it happen 3 hours ago when wearing a dress. I was standing washing dishes and a gush of fluid came out there was a puddle at my feet. Should I be worried. Should I call my ob or family doctor.
