What Causes Persistent Pain At The Nape Of The Neck?

Dull but persistent headache in the lower back of the head/upper neck for 7 straight days. Doesn t respond to tylenol but does seem to get better (temporarily) with ibuprofen. Also, during and after ejaculation, the headache pain is excruciating and lasts for several minutes. Today I have also noticed that the right side of my face/cheek is twitching . I m generally not an alarmist but have some concerns about the headache in combination with involuntary facial movements. Thoughts?
