1. Painful urination can be the result of a
vaginal infection,
urinary tract infection, or inflammation of the urethra or vagina [due to sexual intercourse, or use of douches, soaps, scented toilet paper,
contraceptive sponges, or spermicide.

Bleeding after intercourse is very suggestive of malignancy, so ideally consult a gynecologist for examination [pelvic] , investigation [biopsy, urine examination,pap smear test]
3. both of the above symptoms can occur in case of dyspareunia [pain during intercourse]
4. Treatment and management should be based on the presence of bacteria or other organisms and if no single organism is identified, all you have to is maintain personnel proper hygiene but if the couple [husband/wife] have similar symptoms then both individuals should be tested for sexually transmitted diseases (STDs).
PS. keeping in mind all the cause(s) your attending physician/doctor will consider the possibility of intermittent
urethral infection with chlamydia, an STD, as well as a more obvious urinary tract infection, and then treat with the appropriate antibiotics.
