What Causes Painful Bump In Back Of Neck?

My husband is 6' 3" about 330lbs. He has had a bump in the back of his neck for about 2-3 years now, he states is started out as a pimple which he popped, but continued to come back and grow. At this time he is having tendeeness and pain, it is about the size of an egg, there is no redness, or drainage, I do not see any type of opening, it is from the inside. I am now worried we have no insurance, and have no idea where to begin, at first I though about a dermatologist but I am not sure anymore.
