What Causes Pain In The Scrotum?

Am describing my boyfriends problems, he lives in Ghana: around end of september he started complainig about pain in his scrotum, it was severe for some days before he told me. I sent him to a doctor. He gave him antibiotics and some weird natural suff. It didnt get better so he went back and he got some fungal creme for he saw a rash in that area. Plus he took an urin sample, but it had no growth (bacteria). Later he changed hospital and was sent to their urologist. He gave him antibiotics (Tavanic) and Vitamin E. I remember that I visited him in that period and the whole area around his balls got hot so he cooled it down with ice cubes and he explained that he did that everytime it occured the last couple of weeks. Two weeks later he should come back if it doesnt get better with the antibiotics. Then the doc told him that it can take some time to get fully down and gave him another dose of antibiotics and vitamins. This was in November. Last week he started complaining again and was wondering if it comes from sitting down a lot. He even told me that he feels the pain now and then but not severe. Today he told me that he feels sick and the pain got worse raising to his waist, feeling feverish. He went to hospital again. His temperature is 37.7°C. And they gave him Malaria drugs and paracetamol. I am confused. Do you have any suggestion?

Your boyfriend might be suffering from epididymo-orchitis. And for malaria treatment you should having laboratory proven malaria. Fever may be present in epididymo-orchitis. You should have to go for further investigation as ultra sound of scrotal area and proceed for further treatment.
