What Causes Pain In The Lower Back And Pelvis?

Bad pelvic pain. Bad lower back pain. Horrific periods. Fibroids in uterus. Went to ER doubled over in pain and flooding. Ultra sound showed a 3 cm complex cyst in left ovary. 11 months ago, the ovaries were fine in a previous ultrasound. Almost 49 years of age. White blood count elevated. Liver enzymes elevate from living on Motrin, I presume. Low grade fever. Braca mutation negative. Should I worry about cancer?

General & Family Physician 's  Response
Hello,

Based on your symptoms I would like to rule out endometriosis. The cyst could be filled with blood. The size of the cyst and associated symptoms lead me to believe that there are other (endometriosis) lesions causing the pain.

• Do you smoke or consume alcohol?
• Have you had any surgeries?
• Do you have children?
• Do you have any other medical issues or take any medications other than the motion?
• How many and how big are the fibroids? What kind are they (submucosal for instance)?
• How are your white cell counts like?
• Any discharge in between periods?
• Any fever?
•     Any family history of cancer?

I recommend:
• An OBGYN consult
• An exploratory laparoscopy to check for endometriosis and other lesions
• A medications check to rule out that as a cause for elevated liver enzymes
• Urine analysis
• CT abdomen

Based on this we can arrive at a conclusion. Endometriosis can be treated well in your situation, if that's the case. Cancer is possible but a very small risk and is unlikely based on these features alone. Please consider my recommendations.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Shuba Hariprasad
