What Causes Pain In Hip,knee And Back?

My father is 71 years, and has had M.Bechterew. Forty years ago, he had
tonsilectomy, and the disease had stopped progressing since. But 3 weeks ago,
he had pain in his left hip, and took Diclofenac as the orthopedist suggested.
His blood results are o.k, except the ESR = 70, and CRP=192. The hematologist
gave him SULFASALASINE, but he got nausea and threw up a lot. He doesn't have
other systemic troubles, except stomach problems. The situation has worsend. Now
he is experiencing sever back pain, knee pain, swelling of the fingers, loss of
strength, appetite and he can't sleep. I am an oral surgeon, so I gave him
amp.Neloran 600 mg i.m, amp. Dexametason 4mg i.m, amp. Ketonal i.m. He feels
better, the pain has eased. So what do you suggest we do next?

Best regards

Hi! Welcome to HCM!
brief answer: Your father is suffering from a type of arthritis - known Rheumatoid arthritis with flare ups....
Detailed answer: From the problems listed in your question are due to arthritis
involving multiple joints. Sulfasalazine was used once upon a time in the treatment of rheumatoid arthritis, but due to its side effects and availability of better drugs it is less commonly used. He must undergo the following lab tests: Arthritis profile......
CBC, ESR, C-reactive protein, Rh factor, ASO titre, Anti-DNA antibodies.
Now a days Methotrexate is the treatment of choice for rheumatoid arthritis. For acute flare up of the disease he must take short term course therapy of steroids and pain killers like Indomethacine, Ibuprofen can be effective alongwith Proton pump inhibitors to prevent the gastritis.
