What Causes Lower Abdominal Pain After Partial Hysterectomy?

March of 2014 I had hysterectomy, but ovaries were left. recovered well and no problems until this week. Had severe lower abdomen pain starting on Friday. Went to PCP today with these symptoms and in return sent me for pelvic and TV ultrasound. I have known cysts on L ovary. R ovary measured 20x22x23 mm. L ovary not visualized. recommends CT. Radiologist is calling it Physiologic pelvic ascites and no cul-de-sac mass. What does this mean exactly?

Hallow Dear,

I believe your surgery was performed through an abdominal approach.

Partial or Subtotal hysterectomy followed by sever abdominal pain one year after the surgery can be due to one of the following reasons:

1. Post surgical adhesions in the abdomen wherein the fibrous bands are formed between the organs fixing them to each other. The adhesive bands themselves restrict the movements of the organs, particularly intestines causing intermittent pain in the abdomen. Adhesions amy have associated congestion of pelvic organs which will cause abdominal pain; however it would be a chronic pain. Moreover, adhesions may lead to:
2. Acute or chronic intestinal obstruction - Acute intestinal obstruction is rather acutely severe condition and most of the times needs emergency surgical intervention. X-ray of the abdomen in standing position will reveal multiple (more than 3) fluid levels.
3. Chronic infection in the abdomen with acute exacerbation will cause such pain. However, it will be usually associated with, fever and pus collection in cul-de-sac.
4. Urinary infection also can be a cause of such pain. Urine examination will rule out or clinch the diagnosis.

The description of the ovaries as found in the scan do not seem to be responsible for such pain.

To me, this pain seems to be due to adhesions. If these adhesions have not caused any intestinal obstruction, you may not get any positive finding in the ultrasound or CT scans.

If the pain can not be relieved by conservative methods like pain killers; and if no other pathology is detected, you might have to undergo surgical intervention for adhesiolysis.
