General & Family Physician 's  Response
Hi & Welcome.

I can understand your concern for your brother.
Low platelet count is due to some internal bleeding due to Dengue fever.

After having gone through the history given in your query and the positive NS1 test, it seems to me he is suffering from hemorrhagic (bleeding) dengue fever.
His platelet count is decreasing due to some internal bleeding caused by Dengue fever. He must continue to take platelet infusion, packed cells, IV fluids, to correct the platelet counts.

Hope this answers your query.
