What Causes Lack Of Focus And Difficulty In Concentration?

I have trouble focusing, holding conversations with people, and tend to change the subject quite often I tend to procrastinate and move from project to project without completing anything. I am sometimes forgetful and cannot remember details. Any suggestions?
