Hi, Welcome to Health care magic forum.
You have got kidney transplant 15 years back, now your creatanine level has increased, It may be related to the kidney, but may not be related to the surgery.
The itching may be related to the skin condition, it can be treated separately.
Any how it is advisable to consult a nephrologist for diagnosis and treatment.
Wishing for a quick and complete recovery. Thank you.
