What Causes High Fever Along With Chills And Sore Throat?

My husband has been sick since Sunday so about 72 hours, he really started feeling a little feverish Saturday night. Sunday he had chills, sore throat and high fever, hovering around 103 and spiking at one point to 104, he kept taking medicine to bring it down. Monday he continued to run a fever on and off around 102 and continued to feel fatigue and aches and a sore throat, sometimes spiking to 103. I took him to urgent care on Monday night and they did a strep test and it was negative. They said it was a virus and he needed to let it run its course. This morning Tuesday he still felt terrible with a fever and sore throat, and he began to have diarrhea and vomiting. He has had diarrhea all day and has vomited about 4 times today, he continues to combat a fever and fatigue. I am just concerned as this is an odd virus that it was two days of fever and symptoms before the diarrhea and vomiting even started and he does not seem to be getting better. Should I be concerned and look at taking him back to the doctor or just continue to let it run its course?

1. As strep test was negative and still fever is continuing, consult an ENT Doctor for further examination and management:

i. He will inspect the throat with a good torch ad tongue depressor to look out for following possibilities:

- whether tonsils are red & inflamed with tender tonsillar lymph nodes below the angle of mandible [acute tonsillitis]
- whether entire throat i.e. posterior pharyngeal wall is inflamed [acute pharyngitis]
- are there inflamed rough granules over the posterior pharyngeal wall [granular pharyngitis]
- If he sees a white patch anywhere in the throat he might suspect ' diphtheria'

ii. whatever be the cause, management of them remain the same:
. steam inhalation: 3-4 times a day.
. warm salt water gargles.
. voice rest and rest at home.
. take hot drinks to sip, avoid fried foods, cold drinks, ice cream, oily food
. Antibiotic, in either case, would be azithromycin/erythromycin and syrup ibuprofen and paracetamol for symptomatic relief from pain and fever

PS. If however fever still continues, check with an Infectious disease specialist [as amoebic dysentery is the most common cause and he might give a course of Flagyl or Tinidazole]
