What Causes Headaches With Blurred Vision?

Hello. I've been getting very bad headaches for about six months. Mostly on my right side temple and back of skull. The pressure is so bad at times and I get very sleepy, blurred vision, feeling as though I want to throw up and in the last few months I keep smelling smoke. The smell is very strong. I also have shortness of breath. What could be going on? I would say they are the worst pan( headache) that I've ever felt.
