What Causes Fluctuation In BP Levels Even After Medications?

Hello
My father is at emergency with my mother right now.
He has had HBP for over 40 years, and has been on meds for HBP for that time. Right now he is taking coversyl and metoprolol. Along with all the side effects (dizziness, nausea, etc.) his BP shoots up at different times throughout the day for no apparent reason. Can you shed any light on why it shoots up at odd times? His current doc was unwilling to take him off metoprolol (mainly for angina prevention).
Thank you.
Chantal.
