What Causes Exercise Induced Myocardial Ischemia?

Hello, My father had a TMT test and it says The test is positive for exercise induced reversible myocardial ischemia. We consulted a cardiologist and he asked my father if they get stress or if they get tired when they walk upstairs, my father said no they don t feel uncomfortable while walking. Then doctor said - we can overlook TMT results and no need to worry. Can you please advise to understand this better ? Thanks Dipesh Kapse

General & Family Physician 's  Response
Hi,

TMT positive for reversible myocardial ischemia means there is chance of blockages in the arteries of the heart on stress due to inadequate oxygenation. It should be correlated with ECG and ECHO. If there is absence diabetes or hypertension, it puts you at low risk of cardiac disease. However if there are other factors like smoking, high cholesterol, obesity, bad lifestyle, family history of cardiac disease then further cardiac work up is advisable.

If your father have none of the above mentioned risk factors, repeat a TMT after 3 months. If any abnormal change then we would need further evaluation. If your father have above risk factors, I would suggest to go for CT angiography to rule out any coronary artery blockage.

Avoid excess fast food, smoking, alcohol. Eat more fresh fruits and green leafy vegetables. Do regular jogging and running for 45 to 60 minutes in morning followed by medication.

Hope this may help you. Contact further if follow up needed.

Best regards,
Dr. Sagar
