What Causes Discomfort During Bowel Movement?

During a bowel movement I am feeling discomfort, and afterward, I see blood on the paper tissue, and a small amount in the toilet. A month ago, my Doctor said it was a crack, and it went away for a few weeks using stool softeners. But now it is back. Could you please tell me possible remedies? Thank you.

- Drink plenty of liquids .
- Prefer high fiber diet with green leafy veg , cereals , whole grains , pulses
legumes , organic fresh fruits .
- Bulk forming laxative .
- Sitz bath ( sitting in tub filled with lukewarm water 5 minutes each 3
times a day and additionally after passing stool )
- Analgesic intake with antacids are helpful .
