General & Family Physician 's  Response
Hello dear
Understand your concern
difficulty in the retracting the foreskin in erect penis is probable features of phimosis.
Other causes are: adhesion band/fibrous tissue between frenulum and glans of penis, balanitis(cause inflammation and swelling)

Mild problem can be treated by pulling foreskin gently by applying lidocaine gen and after warm bath.
But exact diagnose should be made by proper penis examination.
Phimosis should be treated by surgery like frenuloplasty or by circumcision under local anesthesia.

So consult the urologist or general surgeon for proper treatment.
Hope this will help you
Best regards
Dr. Sagar
