General Surgeon 's  Response
Hi.
Thanks for your query.
Noted the history of gastric bypass surgery on 4/27, dehydrated since then, no bowel movements in 3 weeks, Lactulose not helping, getting light headedness and dizziness with feeling like passing out.
The commonest causes I can think about are:
As per the history you have provided, this looks to be due to your inability to have proper hydration due to restricted intake or some problems that you have no posted about.
Less the water in body, less will be for the colon to keep the feces liquefied. Hence the constipation and unable to pass stool.
Additional Laxative like creamaffin plus can work better than Lactulose.
Get an X-ray of the abdomen and if there is impaction of feces, you may then need saline and soap water enema to clear the colon so that further bowel movements can occur without much problems.
Sitz bath gives soothing effect.
To increase the oral intake of foods and water.
Consult your Surgeon for further clinical evaluation and plan a proper management.
