What Causes Chronic Cough?

I believe I have a hiatalhernia. I also have acid reflux. Lately I have been coughing a lot and a milky white substance backs up in my mouth after I eat or even drinking water like when taking my pills. It is worse at night and I have to sleep with head elavated. It is becoming hard to breath without coughing alot. Maggie,
