Hi, Welcome to Health Care Magic. It seems like your father has got angina like symptoms. If your father is Diabetic, Hypertensive and Smoker it is more likely he has some sort of heart problem. Angina is cardiac chest pain which is generalized and occurs mostly after exertion and relieves at rest.
ECG shows a stress pattern. Otherwise its very hard to comment on ECG with these parameters as we have to look at the whole 12 lead ECG.
Please repeat an ECG during chest pain. Do a stress test. you can do echo after that its advised by local doctor.
Do following blood tests
-Liver Fuction tests
-Renal Function tests
-LDH
-Creatinine phosphokinase
-Complete Blood count

Use tab loprin(coated aspirin) 75mg and tab rosuvastatin 10mg both once daily
