For blurred vision
1) get your refraction done and wear a correct power spectacle
2) complete eye check up is needed to rule out treatable cause of
cause of blindness(eg cataract )
Hope you are not using atropine (dhatura)in any form which can cause a) dilated pupils and dryness in eyes and hence blurred
vision
b) dryness of mouth
c) constipation

For difficulty in swallowing
since you said you have muscle weakness in other part of body so it is advisable to rule out myaesthenia gravis if you have following symptoms
a) fatigue n tiredness at end of day or with minimal activity
b) difficulty in speaking
c) shortness of breath with minimum walking or working

Loss of hearing could be age related or due to impacted wax.its better to see ENT doctor for that.
Hope this helps.
