What Causes Bloating With Smelly Urine And Muscle Fatigue?

Hi, I have extreme bloating, in the morning, when i dont eat, and obviously ten times worse when i eat anything. so much that i can barely eat. My urine is very dark and strong smelling, I drink only water, and about 2-4 litres a day. I am unable to lose weight and i weigh about 12kgs above my normal weight. I go to the gym 5 days a week, walk home from work 4 nights a week (7km). I am at my wits end trying to figure out whats wrong with me. When i eat carbohydrates i gain even more weight, its as if im insulin resistant. So i stay away from heavy carb foods, but now im finding im quite depressed and down unless i eat a bit of sugar. Then im back to the happy bubbly person i used to be. I have extremely bad breath even though i have great oral hygene. I have muscle fatigue, a sore neck all the time, and very bad circulation. If im working out on a machine my feet and legs go dead, the same if im chilling out and have my legs resting high against something, i get bad pins and needles first, almost instantly, then they go completely dead. I have a horrible spotty rash aswell around my mouth and nose. Any answers are greatly appreciated, thanks.

Noted the history and understood your concerns.
You have multiple problems as you have enumerated and may thus be connected to a general cause like diabetes, hypothyroidism and so on which needs the following:
In depth clinical evaluation, examination by a Physician and thus get the relevant tests or investigations which may be as follows:
Tests of blood: CBC, thyroid, liver, kidney functions; tests for diabetes and anything else as may be guided by the clinical evaluation.
Bloating can be due to intestinal infection wherein there is outgrowth of bacteria producing gases or intestinal obstruction.
This can be diagnosed by the stool tests for three consecutive days.
Strong smelly urine in spite of plenty of oral water and fluids can be due to kidney disease or urinary tract infection. Get urine tests for routine, microscopy, culture and sensitivity.
If all the tests indicate an infection, get a proper course of an antibiotic, metronidazole and probiotics.
Stop aerated drinks and avoid all the foods and beverages that instigate or enhance the symptoms.
Ultrasonography, CT scan of the abdomen may help further in proper diagnosis and management.
