What Causes An Adrenal Fatigue?

Thinking I have adrenal fatigue. I had a 12-hr saliva adrenal test and levels proved to be low so i was put on a low dose of hydrocortisone (10mg daily) which helped my energy initially - 5 months ago. I am still feeling fatigue which comes and goes, also depression and trouble sleeping, numbness and tingling in hands and arms sometimes. Can't do high intensity work outs or I'm tired. In Aug. '14 I had a total hysterectomy because of endometriosis and PCOS. Thinking the adrenal fatigue was caused by that. My question: Should i get a second opinion? Do you think it could be something else?
