I have recently experienced some severe heartburn. Also the hiccups and the need to burp have surfaced as well. Twice I have begun to eat, but the discomfort increased and caused somewhat of a gag reflex. I have had significant phlegm come out. I have had some mid-back issues as well. I know the mid-back nerves can have a significant impact on the digestive system. Not sure if they are related. Thanks.
Looking at your description it might be related to stomach hyperacidity leading to gastritis or stomach ulcer which on irritation may cause spasm and gag reflex and hicups as well. Mid back pain might be associated with gastric or pancreatic disease. I would advise you for serum lipase and serum amylase tests along with liver function test, an ultrasound abdomen and an upper gastrointestinal endoscopy to establish a diagnosis and further treatment.
Meanwhile you should take antiacidity medicine like pantoprazole or ranitidine group of drugs with antacids or sucralfate syrup for symptomatic relief from heartburn and further aggravation of the existing condition.
