General Surgeon 's  Response
Hi.
Thanks for your query.
Noted the history stomach infection, anal fissure, sentinel tag, started on phylum husk and cremaffin followed by loose frequent stools.
The history you have provided indicates that the problem started with intestinal infection. This must have lead to the anal fissure and sentinel tag.
This means that the fissure was due to intestinal infection and not the hard stool as the history is missing.
Hence there is no point in continuing the Phylum husk and creamaffin.
I think stopping these two things along with the other care as follows will solve your problem:
Sitz bath.
A course of an antibiotic and metronidazole along with symptomatic and supportive treatment like probiotics, activated charcoal should help you the maximum.
Only if the medical management fails, that you have to go for surgery; consult a General Surgeon for ano-proctoscopy.
