Hi welcome to HCM.
Significant fluctuations in heart rate can be secondary to complete heart block. This can reliably be diagnosed through continuous 24 hr HolterEKG.Electrophysiological studies can help to accurately determine the fluctuations.Drugs like beta blockers and amiodarone can help to normalize the heart rate.Pace makers can be implanted for timely use.Consult your cardiologist for further queries.
Thank you
