What Causes A Flexible Belly Button And Breathlessness?

my son has an inny belly button when he sits in certain positions or lays down, but an outie belly button with he stands upright. Recently he feels like he cannot get enough air and needs to breathe very deeply repeatedly in different positions - could a hernia cause these symptoms?
