Since you have lung infection in past this white lesion can be old healed lesion of past pneumonia. Or it csn be a new infection with TB or pneumonia. Since you are past smoker and old age, chances of malignancy are high. So better to under go CT thorax with contrast. This will solve all your query. And yes you should also go for sputum examination for tb. Also bring old x rays for comperision of lesion size. So go for CT thorax with contrast.
