Hello
Pain in the right lower quadrant at a fix point with feeling of a lump is an indication of chronic appendicitis . Diagnosis can be confirmed by ultrasound .
While as you mentioned that there is low grade fever , nausea , lower abdominal pain , so may be due to these reasons.
1 Salpingitis i.e. infection of right fallopian tube may be the reason of such fever . Diagnosis can be confirmed by ultrasound as well physical examination by a gynaecologist.
2 Right renal calculus or urinary tract infection may be the another cause of such symptoms .
As you mentioned in the query that often there are urinary tract infection present , so get in urine culture and drug sensitivity test .
After culture take proper antibiotics as prescribed by doctor.
3 Typhoid fever may be the another reason of such fever . Get in Widal test as well serology test for confirmation . Typhoid fever is an intestinal fever , so pain may be due to typhoid .
Deal as mentioned above .
