Typhi H titres to be significant i.e., 1:320 but other titres are borderline they can be elevated in other non specific fevers unrelated to Typhoid also (this is called Anamnestic Reaction). But to confirm Typhoid fever blood cultures are gold standard if taken within 2 wks of symptom onset preferably in first week. Regarding widal test you can increase the specificity of this test by getting a repeat test after atleast 1 week preferably 2 weeks. Rising titres are more reliable to diagnose typhoid rather than single titre value. Coming to treatment there good antibiotics are available now for oral therapy. Apart from antibiotic therapy, food, water hygiene have to be concentrated along with healthy diet, adequate rest. Please follow your treating physicians advice for Antibiotic therapy.
