Hello and welcome to HCM
Your report has suggested that you are suffering from Polycystic ovarian syndrome (PCOS).
In a ovary, during a monthly ovarian cycle multiple follicles start maturing. Out of these multiple follicles, only one matures and is released every month in a process called 'Ovulation'.

In PCOS, the follicles are present in the ovary but none of them mature and none are released during ovulation.
Absence of ovulation causes failure of fertilization and thus pregnancy.

For PCOS, medical treatment is required which includes Metformin and Clomiphene citrate.
Metformin has insulin like effect on body and clomiphene citrate stimulates ovulation and improves chances of fertilization.

The other problem that you have mentioned- Fallopian tube blockage needs operative measures to open the blocade. Fallopian tube blockage impairs the transport of ovum/ egg from ovary to the uterus. So, even if PCOS gets treated this blockage will not allow fertilization to take place.

Thanks and take care
