Taking Diltiazem ER Daily After Ileostomy. Not Getting Benefited By Medicine. Suffered From Stroke Earlier.BP Under Control

My husband takes diltiazem ER 180 mg caps (once daily). He had an ileostomy March 2013. Since then he finds his cap in the pouch, not dissolved. He is concerned about not receiving the benefit from the med. He also has had 2 strokes previous and must be in control of his blood pressure. Do you have any help with this problem?

Hi there,

Thanks for your query.

ER stands for Extended Release. That is, the drug is released from the capsule very slowly and steadily so as to maintain its effect for 24 hours. However, the forward-moving action of the intestine propels it through the intestine in normal time. It reaches the pouch, BEFORE the capsule has released less than 25% of its contents of drug.

To circumvent this problem, a normal salt of diltiazem in TABLET FORM, should be used. This way, the tablet would dissolve, release the drug COMPLETELY and absorbed in the blood, MUCH BEFORE it reaches the ileostomy site.

Consult your doctor and apprise him of my opinion. I am certain that he agree with me and will prescribe normal diltiazem or another alternative drug.
