I have gone through your query regarding acute pain in ear in journey of mountains and your ENT doctor has diagnose it as ASOM causing retraction of ear drum. In your last attack of such you were benifited by antibiotic amoxicillin and ceflexin. Unfortunately your wife has again develop same problem in in journey of hilly area. As far as treatment is concern I would like to say that all such types of problems usually arises when your eustachian tube is not working properly most likely due to cold or sore throat.
