Thanks for writing to H.C.M. for your query.
I have gone through your query regarding one node below your right ear, which is not responding to treatment with antibiotics, as you said that you have sinusitis, this maybe one reason for this node.
Here I would like to tell you that enlargement of lymph node below and behind the ear may be due to any infective or malignant pathology of it's draining area i.e.- Nose and paranasal sinuses, tongue and teeth, jaw bone, throat and ear. Therefore to treat any node one should find out primary lesion some times we can find out it clinically but some times it is identified by investigations.
I would like to suggest you few basic investigations.
(1) C.T. scan of paranasal sinuses.
(2) F.N.A.C. from lymph node.
(3) Direct Laryngoscopy.
Thorough examination of oral cavity, Pharynx and larynx and their co-relation with inferences of investigations will disclose the diagnosis which can be later plan for treatment.

With Regards.
