Suggest Treatment To Lower Triglyceride And Cholesterol Level

hello doctor
my father is 47 years old and his Triglyceride was found to be 612, also the cholesterol came out to be 208. 5-6 years back he use to take medicine atorvastatin 10 mg...which reduces the TG to ~300 (5 years back TG was 550) and cholesterol to below normal (5 years back CH normal).
1 year back he has stopped the medication.
doctors after seing TG levels asked him to undergo TMT and ECHO tests. Echo came out to be normal but TMT says "the exercise strees test +ve for provacable myocardial ischemia". protocol duration 7.5 min. 85% of MPHR 146.
now doctor advised him to go for angiography. also he has a family history for TGs. and Angina

he doesn't have any sort of pain while doing any vigorous exercises or while climbing stairs (4th floor) or while walking..
just have his TGs to abnormally raised.
kindly advice...

As per the history provided, your father is 47 yrs old suffering from dyslipidemia (Triglyceride 612, Total cholesterol 208) and Ischemic heart disease (probable coronary artery disease, hence advised for Coronary angiography). I would like to know whether he is suffering from other conditions like Diabetes mellitus (which can worsen the TG levels) or hypothyroidism.
It is advisable to lower the TG levels to 1. Avoid intake of alcohol, saturated fat and refined carbohydrate. Also if he is a smoker it needs to be stopped.
2. Drugs of the statin group need to be started (like Atorvastatin or Rosuvastatin) to lower cholesterol and a fenofibrate may be added for additional effects on Triglyceride levels.
3. However, when an individual is taking a combination of these two drugs, the liver function test and CPK levels need to be monitored at regular intervals. Muscle pain may be experienced, which needs immediate attention.
4. If the blood sugar and thyroid profiles are not done, it is better to rule out any abnormalities in these parameters.
5. In case any central abdominal pain develops, suspicion of Pancreatitis needs to be kept in mind (due to hypertriglyceridemia)
I would ask you to consult your Cardiologist as soon as possible, so that these medications can be started under supervision.
