General & Family Physician 's  Response
Thrush is common in immune-compromised patient such as HIV positivity, persons taking steroids, malnutrition and organ transplant patient. As you have history of unprotected sex, you must get tested for all serious sexually transmitted infections.
HIV screening is done by ELISA test and confirmation is done by Western blot test.
Advice
1.     Fluconazole 50 mg daily per oral for 14 days
2.     Do gargle with chlorhexidine mouth wash every 8 hours
3.     Do not rub thrush with brush as it will cause bleeding and infection
4.     Take hepatitis B vaccine on 0, 1 and 6 months – 3 doses for future prevention of hepatitis B
5.     Always use condom while having sex with escort
6.     Investigations
a.     HIV screening
b.     Australia antigen
c.     HCV DNA
d.     VDRL
