General Surgeon 's  Response
Welcome to healthcare magic
I will help you with your problem

I have gone through your question. to recapitulate there is pain in the testis, and its very swollen. presently using betamathosone-clotrimazole. and he is also due for circumcision.
I need answer to my following question?
- duration of swelling?
- can it trans-illuminate by placing a torch on effected testis in dark?
- any accompanying fever or the swelling is tender?
- any investigation for the problem?

Coming to the point it appears to be infected hydrocele. Ultrasonography scrotum along with blood complete picture is needed to reach a diagnosis. betamethasone is steroid and clotrimazole is anti-fungal usually given for skin condition. I don't think it is useful in your case. I recommend to visit a general surgeon who will illicit a history and examine it. Infected hydrocele is a easily treatable disease and your husband will fine in matter of days. wishing him rapid recovery.

I hope it will help you. Do rate my answer if you like it.
