Suggest Treatment For Testicular Pain With Painful Ejaculation

Hi, I'm a 41 yr old male, who herinated L5/S1 @ 18yrs old in workplace accident...imaging confirmed herniation, as well as partial lumbarization @ S1 left side, transitional vertebrae with elongated tvp @L5/S1...fusion considered in 1988 etc but had tvp excised in 1990...now DDD, chronic pain, multiple level disc bulges, sciatica etc...could these issues perpetuate symptoms of Pudendal Neuralgia...latest MRI confirms indentation of thecal sac from central L4 disc bulge etc...suffering now from painful ejaculation, constant ache in groin region and pain in testicles similar to a bad case of the "blue balls"...trying to get some insight..CT scan and blood work negative for any other disorders etc...

With the history you have given, the pain in the testicle and groin can be explainef well to ne due to herniated discs and related changes causing pudendal nerve compression or so.
I think this is high time that you get fresh MRI done, clinical evaluation and surgery done.
