Suggest Treatment For Swollen Knuckles

I have very swollen hands especially between the knuckles. Not tender to touch but swelling never goes down and burns like you wouldn t believe especially at night time. Doc has no idea what s causing it. Says it s not fluid retention and bloods show no sign of inflammation. Can you offer any guidance please.
