hi doctor My name is Ekta and I am 32 yrs old. my height is 5 and weight is 88. I have been married for 5 yrs and have no kids. I am a PCOD patient. earlier my cycles use to come in 35 to 45 days without any medicine and I used to have approx 10 cycles a year. Now i keep on waiting for the cycle for 50 to 60 days then as told my the doc I take meprate twice a day for 5 days and then wait for another 5 days for the ycle to come. I have taken meprate sometimes in past but now consecutively for 3 months I am taking Meprate AFTER 50 55 days of waiting. my major concern is to regulate my cycles first and then to concieve. First 3.5 yrs of my marriage got wasted as I was not ale to consummate my wedding due to fear factors in me. meanwhile i have visited many doctors and got several tests done. they have even guided me for artficial insemination but all went in vain. can u please help and show me the path

General & Family Physician 's  Response
Hi Ekta,
Thanks for posting in HCM.
I understand your concern.
Treatment for 'polycystic ovary syndrome' (PCOS) focuses on management infertility caused due to irregular periods and ovulation.
The following things needs to be done to overcome most of your problems.
1. Lifestyle changes by weight loss through a low-calorie diet combined with moderate exercise.
2. Regulation of menstrual cycle by use of estrogen and progestin combined birth control pills.
3. Regulation of ovulation and menstrual cycle by taking oral hypoglycemic drug metformin.
4. Induction of ovulation by administration of clomiphene, if necessary.
5. Reduce excessive hair growth by birth control pills to decrease androgen production or by another medication called spironolactone.
With these medications, it should help you to improve the chances of conception. It takes quite a few cycles of treatment and regular follow-up to conceive and hence you need to have patience.
Kindly consult infertility specialist in this regard for appropriate management.
Hope the information provided would helpful.
All the best.
