General Surgeon 's  Response
Hi.
Thanks for your query.
Noted the history of you being an obese person of 68 years, having gaseous distension to the extent that it put pressure on diaphragm and respiration difficult and also causes pain in abdomen.
The causes can be as follows:
Aerophagia due to stress, anxiety or inactivity.
Fast eating and swallowing.
Aerated drinks.
Consumption of foods and beverages that causes gases.
Sub-acute intestinal obstruction.I would advise you the following.
X-ray of abdomen in standing position.
Ultrasonography of abdomen
Enterography/ enteroclysis by barium or or CT scan.
Tests of blood, urine and stool.
Clinical evaluation by a General Surgeon
This will give you the diagnosis and treatment.
