OBGYN, Maternal and Fetal Medicine 's  Response
Hi dear,
I have gone through your question and understand your concerns.
Severe pain during periods is most likely due to adenomyosis, the signs of which are present in the ultrasound.
Adenomyosis occurs in 35-45 years of age and is characterized by dysmenorrhea and pelvic pain.
Evening sweats are unrelated to this pain.
I will suggest you to consult an OBGYN specialist to get properly examined and treated accordingly.
Hormonal medications are present to treat this condition.
Hope you found the answer helpful.
