Suggest Treatment For Severe Itching In The Genitals

Hi, I am 29 years old. I have itching on my genitals for last 5-6 years and the severity would range from low to medium ( mostly due to sweating in that area). I used to apply itch guard for a few days and it used to get okay. Now, for past 4-5 months i have severe itching on my genitals (penis, testicles and the area below ) , I use condiderm powder frequently. But when the problem increases I apply Quandriderm ointment at night, for 2-3 days, after which its okay for a few days. There is also lot of flaking and cracking of skin. (attaching the pics) I usually end up itching while sleeping also. I don t have this itching anywhere else on my body & my job mostly entails sitting in AC. Also, i have been using cotton boxers after this problem started. Please advise : what can be the problem & what medications & precautions should i take

Welcome to HCM and thank you for asking

In yours case, based on your symptoms, you are suffering from tinea cruris or jock itch as it is differently called caused from a fungal infection of the groin.The conditions fungi like best are warm, moist and airless areas of skin, such as the groin.
Excessive sweating around your genitals is stimulated also from your job(as you refered to me). It can further lead to rashes, itching and peeling of the skin in the area around your groin and this is caused due to the growth of a fungus in this area.

First I suggest to you some advices:
1.Since jock itch is contagious, it is extremely important to observe a strict hygiene routine
2. excessive sweating, bad odor and jock itch can be treated by improving your hygiene routine, bathing at least twice a day and continue to wear cotton undergarments that are not tight around the groins. If necessary change your underwear twice or thrice a day.
3.Ensure that your groin is dry by wiping it carefully with a towel or even using a blow dryer to get rid of the moisture created by the sweat.(this prevents fungus to develop)
4. You should drink a lot of water and fluids to keep your body temperature low and decrease sweating. Green tea, cider vinegar mixed with 1 tsp of honey and garlic is excellent for excessive sweating and should be used in food preparations.
5. Processed, spicy and greasy foods which convert into sugar in the body should also be avoided.
6.You should also avoid chain-smoking and excessive consumption of alcohol.

Treatment:
1. Antifungal cream- There are various types and brands - for example: terbinafine, clotrimazole, econazole, ketoconazole and miconazole. These creams are good at clearing fungal skin infections.
Apply the cream to the surrounding 4-6 cm of normal skin in addition to the rash for as long as advised
2. For skin that is particularly inflamed, your doctor may prescribe an antifungal cream combined with a mild steroid cream. This would normally be used for no more than seven days. You may need to continue with an antifungal cream alone for a time afterwards.

Please remember !! It's very important for you to know that the steroid of course reduces inflammation and may ease itch and redness quickly BUT, the steroid does not kill the fungus and so a steroid cream alone should not be used.( I'm explaining this to you because you told me that you've used Quandriderm and the main ingredient is betamethazone, which is a steroid)

SO, I also recommend to you to stop applyng all these creams , but to consult a dermatologist in order to determine the final diagnose.
