Suggest Treatment For Prostatomegaly
My father has been diagonised with sever prostatomegaly with post void urone 500cc and moderate bilatetal hydroureteronephrosis.during first vist his creatine was 3.19 and toatl psa 27.now he is on catherel and after 15 days the creatine is 1.4 and total psa is 20.should we go for surgery or do waitful watch

Thanks for query ,Based on the facts that you have posted your father has Chronic Retention of urine due to grossly enlarged prostate with impending renal failure .
You need to consult qualified Urologist for evaluation and get following basic tests done to decide further line of treatment .
1) Urine culture and take appropriate antibiotics.
2) Repeat Serum PSA after 3 weeks
3) He needs long term catheterization to regain his bladder tone which needs to be confirmed by doing Cystometric study of bladder
4) Once he regains his bladder tone which may take 4-6 months should be followed by endoscopic resection of Prostate (TURP).

I have gone through your question. I can understand your concern.
Your father has large sized prostate. It can be due to benign prostatic hyperplasia or prostate cancer. His PSA level is also high. He should go for TRUS guided prostate biopsy. It will give you exact diagnosis. Then he should take treatment accordingly.
