Suggest Treatment For Proper Bowel Movement

I have twins sons and one of the boys is potty trained and has no bowel issues whatsoever. The other is afraid to "pee" and "poop". He cried out in pain most days when he tries to push out the black tarry poop. He actually seems to have a continued ooze of the black tarry substance every day... he's uncomfortable and is very scared when he feels his stomach getting upset or feels like he is going to have a bowel movement.

In the recent past (within one year) he has had a bowel movement where his internal organs actually came out of his bottom. We rushed him into the doctor and it did go back in on it's own... but am concerned that this may continue to be an ongoing problem for him. What is your advice on next steps or what this could be? Thank you!
