Hi,
read and understood that you and your wife are trying to have a baby and went to the Gynecologist who on examination and rather the lab tests told you that you have low sperm counts and it appears thatthe Gynecologist gave you the medicines for the increase in sperm count.
As you may know and would realize that the Gynecologist is not a competent authority to know about the sperms.
Hence ideally you should visit an Urologist who can examine you directly to see for the local causes such as varicocele, or so and would advise you the further tests of repeat sperm counts after an abstinence of at least 5 days.

Further tests of color doppler and ultrasonography need to be done and then the decision will be taken whether you need a medical treatment or need some surgical intervention in such cases such as varicocelectomy.

I hope this answer helps you.
