Orthopaedic Surgeon 's  Response
Gout is a form of crystal synovitis common in males after age 20 and is rare in women until menopause.
Since you have not mentioned which antiurthemic medications you were on, i can't comment much on that but i want to know what you were prescribed.
At present should have some form of dietary restriction ( limit legumes and mushrooms also meat , poultry and seafood). Also limit or avoid meat extracts and alcohol for bringing uric acid levels down. You should also start quadriceps strngthening exercises for knee pain.
Please continue your antidiabetic and antihypertensive medication

Get serum uric acid levels done monthly.
Please follow up with serum uric acid and X rays of B/L knees as i would like to know the extent of arthritis.
Also please get the x rays of the involved leg where tophi is present as it might need surgical removal.
